The camera can not have enough time to properly focus or process the image between scans with high-speed scanning. Please slow down the scanning speed. Ensure the environment is well-lit when scanning. Poor lighting can affect the camera's ability to accurately capture the QR code, especially when scanning rapidly. If the QR codes are of low quality, blurry, or distorted, the camera will struggle to read them, particularly when moving the phone too quickly.
